Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary

The Wraparound Facilitator provides care planning for youth that builds on the collective action of a committed group of family, friends, community, professional, and cross-system supports mobilizing resources and talents from a variety of sources resulting in the creation of a plan of care that is the best fit between the family vision and story, team mission, strengths, needs, and strategies. The Wraparound Facilitator utilizes a holistic, strengths based, and family driven treatment approach to facilitate collaboration between the youth/family and all parties involved in the client's treatment, to ensure the youth/family's success and longer-term stability. The Wraparound Facilitator encourages the team to utilize out-of-the box thinking to develop creative solutions through a strengths-based, family-driven, team-supported, and a culturally competent process.

Essential Functions and Responsibilities

  • Performs assessment of youth/family needs through the development of a multi-perspective family story/timeline
  • Guides the family through the wraparound process and implements the four phases of wraparound with fidelity to the model and empowers families to use their voice in a team based approach; four phases include: Engagement & Team Preparation, Initial Plan Development, Plan Implementation and Transition
  • Builds and develops child and family teams, facilitating monthly child and family team meetings and additional crisis meetings as necessary while maintaining necessary documentation surrounding those meetings
  • Completes Care Plan, Crisis Plan, Intervention Plan, Plan Service Authorizations, Minimum Data Set, Eligibility Reviews and Participant Status within the TOBI system and uploads all Monthly Child and Family Treatment Team Meeting minutes, Family Attestations, Pick Lists, CANS, Transition Plans and justification/alteration comments within the TOBI system so DMHA can review on behalf of clients
  • Reviews and incorporates DMHA Site Coach Feedback and guidelines and disseminates the coaching feedback to the team in an effort to increase the wraparound fidelity to the model
  • Advocates for youths and their families through community agencies, schools, and court systems while adhering to the 10 Wraparound Principles: Family Voice and Choice, Team Based, Natural Supports, Collaboration, Community-Based, Culturally Competent, Individualized, Strengths-Based, Persistence, and Outcome-Based
  • Utilizes a flexible and holistic approach to meet families 'where they are at'; may be required to travel outside of Marion County to facilitate team meetings
  • Completes regular assessments utilizing the CANS Assessment tool and corresponding documents
  • Develops comprehensive transition plans with discussion occurring in the team development phase to ensure adequate natural supports/family voice for long-term success
  • Monitors multiple providers to ensure fidelity to the model, adherence to program guidelines, and to provide coaching feedback when necessary; supervises and manages providers to ensure work with families is agreed upon within team setting and included in the plan of care and approved by DMHA; reviews provider monthly reports to ensure efficacy and timeliness
  • Maintains updated treatment plans with progress note timeliness per agency guidelines; complete other necessary documents within the EPIC system
  • Completes monthly client check within the Indiana Medicaid Portal to verify program eligibility and status

Job Requirements

  • Clear Department of Child Service abuse history
  • Clear federal, state and local background history with finger prints
  • Clear drug screen
  • Valid Indiana driver's license that meets Eskenazi Health's driving policy standards; valid vehicle registration and insurance must be maintained
  • Maintain eligibility to provide items or services payable by federal health care programs

Knowledge, Skills & Abilities

  • Bachelor's degree, with two or more years of clinical experience; or
  • Master's degree in Social Work, Psychology, Counseling, Nursing, or a related field, with two years of clinical experience. (clinical experience includes: case management, skill building, child welfare, juvenile justice, and educator K-12)
  • Maintain qualifications and obtain DMHA active approval status as a Service Provider
  • CANS Super User certification with Yearly Booster Training
  • CPR certification
  • CPI certification
  • CMHW Service Provider Orientation Training and Indiana Strengthening Our Communities (IN SOC) webinar certificates
  • Yearly Wraparound Booster Training
  • Annual education hours and or CEU hours
  • Indiana Engagement in Wraparound Training
  • Indiana Introduction to wraparound Training
  • Indiana Intermediate Wraparound Training
  • Indiana Advanced Wraparound Training
  • Acquire or working toward acquiring Wraparound Practitioner Certification
  • Employee must be self-motivated with the ability to think quickly in an effort to respond and/or work with teams to respond to potential crisis situations
