Responsibilities

Conviva Care Solutions is seeking a hard-working and dedicated Registered Nurse to promote and restore patients' health by completing the nursing process and who is interested in being part of a team that focuses on excellent service to others.

Essential Functions:

• Identifies patient care requirements by establishing personal rapport with patients Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  
• Responsible for l evaluation ofpatients with ambulatory diagnosesas well as possible acute conditions. The triage nurse will work in partnership with the center's mid-levels and PCPs.
  
• In consultation with the center's mid-levels and / or PCPs the triage nurse will initiate treatment if necessary, for common ambulatory diagnoses.
  
• Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
  
• Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  
• Resolves patient problems and needs by utilizing multidisciplinary team strategies.
  
• Work in other duties as assigned
  

Position Requirements:

• Must be willing and able to perform the following procedures or at least be willing to be trained to do them:
  
  • Starting and IV and initiating IV fluids
    
  • Give Injections
    
  • Simple wound care
    
  • Insert a foley catheter if appropriate
    
  • Administer aerosol treatments
    
  • Perform EKGs
    
• Good communication skills
  
• Works well in a team of healthcare providers.
  

Education:

• Associates Degree or Bachelor of Science Degree in Nursing
  
• Nursing Diploma
  

Working hours: Monday to Friday 8AM-5PM
