Job Summary:

Provide support and assistance to LIFE St. Francis participants and potential participants related to transportation activities; maintain, communication with driver during all transport activities.

What your day will look like:

Full-Time,Monday - Friday 40 hour work week

You will be responsible for:

*Assisting participants and potential participants during transportation. Assist participant with necessary preparation ( such as putting on outer garments, locking doors, carrying items, helping with assistive devices, etc. ) prior to transportation.

*Provide door to door assistance to participant or potential participant onto and off van in accordance with LIFE St Francis policies and safety procedures.

*Assists the driver to secure participant personal items and DME. Remains on van while van is transporting participants.

*Acts as primary communicator with center, participant, and support services when vehicle is in transport. Accompanies participants during outside appointments.

*Observe, report and record changes in the participant's condition and/or environment to appropriate staff as needed. Is alert to signs and symptoms of pain or discomfort, Asks questions if participant can make their needs known, report pain or discomfort to transportation coordinator.

*Takes measures to provide safe care to prevent injury to self and participants during transportation. Uses proper body mechanics. Adheres to accepted infection control practices. Maintains a clean, safe work environment. Provides information to LIFE St. Francis Interdisciplinary Team as needed.

* Is an active, committed member of LIFE St Francis staff and participates in program activities as assigned including but not limited to, interdisciplinary team meetings, family meetings, staff meetings, in-service and training programs, daily briefing

Qualifications:

Certification/Licensure: Driver's license desirable

NJ Certified Home Health Aide certification a plus.

Education - High School diploma or equivalent. Requires sixth (6) grade English reading comprehension level; Writing and math skill level to the extent necessary for the successful and safe performance of required job functions.

Education/Licensure:

Experience: Must be at least eighteen (18) years of age;

One year of experience required with frail elderly population in acute, long term care, or community based setting preferred
