Description:

The primary purpose of the job position is to serve as a resource for individuals looking for assistance in understanding and navigating senior living options and guidance in obtaining services. This is a temporary position as this is grant funded.

Receive new referral leads generated by 518 Seniors and obtain initial information relating to clinical and financial needs. Assist individuals interested in admission information to Eddy Senior Living (ESL), including independent living, assisted living, or memory care. Provide referral information for additional resources offering senior services by SPHP and its affiliates, as well as provide recommendations outside our network as appropriate. Assign housing referrals to ESL Sales Team members through Enquire CRM. Promote Eddy Senior Living to potential residents and/or their representatives and to community agencies.

ESSENTIAL FUNCTIONS:

  • Establish and maintain positive relationships with prospects and/or their representatives to provide information about Senior Living Services in the Capital District and more specifically about Eddy Senior Living Communities. Obtain demographic, financial, and referral information to determine eligibility and interest. Refer individuals to appropriate resources as necessary
  • Serves as a central resource for individuals who contact 518-Seniors needing help in understanding the various levels of care as regulated in New York State, associated criteria, and services (including independent, assisted living, enhanced care, enriched care, and memory care).
  • Help to identify specific needs of the client and to guide them through the process of learning about senior living options, resources available, referrals to specific communities, scheduling tours, with a warm hand-off to the appropriate senior living specialist (salesperson).
  • Coordinate non-paid referrals in Enquire database (qualified and unqualified) including referral source. Document qualified referrals in Enquire in a timely manner and provide a warm hand-off to a senior living specialist if appropriate.
  • Assist with lead generation by establishing and maintaining a directory of professional companies, organizations, and contacts with accurate information within Enquire database. Develop positive working relationships with non-paid referral sources to regularly discuss prospective ESL candidates and to ensure a clear understanding of ESL services, levels of care, and benefits provided by our communities.
  • Communicate regularly with Eddy Senior Living and other SPHP services representatives to share key information and provide a seamless transition for services as needed.
  • Create and disseminate reports (weekly referral/admission updates and others as required). Use of various computer programs for tracking and reporting purposes.
  • Maintain working knowledge of ESL levels of care as well as SPHP and surrounding community senior services. Maintain contact list for resources to consult or refer to related to non-ESL services.
  • Other duties as assigned.

MINIMUM REQUIREMENTS:

MENTAL/VISUAL/PHYSICAL DEMANDS:
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
  • Must be able to handle information in a confidential manner at all times.
  • Must be able to read, write, and speak fluent English.
  • Pleasant, outgoing personality is essential.
  • Good verbal and written communication skills are necessary.
  • Must be organized and detail-oriented with the ability to follow jobs to completion.
  • Must have operational knowledge of photocopier, FAX, postage meter machines.
  • Must be proficient in the accurate use of computer software including word processing, spreadsheet, and database programs.
  • Must have the manual dexterity to use the telephone and office equipment.
  • Position may require the use of repetitive action, forearm rotational movement, writing, simple grasping, fine manipulating, pushing, and pulling.
  • Physical effort includes walking, standing, bending, reaching above shoulders, and lifting up to 30 pounds.

Education / Experience:
  • Two-year degree required, or four years related healthcare or senior living sales experience.
