Description:

Assesses learning needs, readiness to learn and actual or potential barriers to learning. Incorporates education into daily patient activity (i.e. medication, equipment, food-drug interactions). Individualizes and documents educational plan based on age, cultural and spiritual values.
Utilizes the information management system to enhance communication. Documents assessment / reassessment, plan, interventions and outcomes according to policy. Documents post-op, transfer and discharge assessment.
Communicates and documents change in-patients condition to all appropriate individuals. Communicates transfer of patient responsibility (i.e. shift report guided by the plan of care or any other pertinent documents)
Documents emergency interventions according to policy. Documents medication use and monitors its effects. Completes admission assessment according to policy. Assesses patient for potential risk factors. Recognizes and integrates patient special needs and individualizes care. Includes pain management and supportive care issues in the plan of care. Adheres to infection control standards specific to patient care. Assumes accountability for managing the delivery of patient care to their assigned group of patients. Responds effectively in an emergency situation so that the patient's immediate needs are addressed.
Prioritizes and organizes work assignment and adjusts priorities based on changing situations. Acts as a patient advocate and uphold patient's rights. Promotes and maintains a safe, clean and orderly environment. Consults and refers to other Health Care professionals.
Refers ethical concerns to appropriate resources. Delegates work to the care team. Plans delivery of care according to patient acuity and staff competencies. Performs as a preceptor when necessary.
Follows clinical policies, i.e. weighing of patients, medication administration, etc. Provides wound care and maintains drainage tubes Initiates and maintains venous access
Demonstrates ability to perform 12 Lead EKG's Interprets dysrhythmias
