Description:

Judge Healthcare is currently seeking a Care Manager, RN to collaborate with patients, family/significant others, peers, physicians and other members of the interdisciplinary healthcare team in the delivery of healthcare services to patients.

This is a part time opportunity! Saturday & Sunday 8a to 8p.

This job will have the following responsibilities:

• Understands and adheres to Performance Standards, Policies and Behaviors
• Completes a comprehensive initial assessments
• Performs all activities for multidisciplinary care coordination at the intra-hospital level of care and inter-hospital level of care
• Performs all transitional planning activities
• Communicates and collaborates all relevant patient information to appropriate health team members
• Documents per department requirements

Qualifications & Requirements:

• BSN or related Bachelor's degree
• 5 years clinical ED, critical care or med/surg experience
• Previous case management experience preferred
• Knowledge of healthcare financial, regulatory, and payer issues
• Computer skills essential
