About the job:

The Registered Nurse provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant's condition to primary caregivers, physician and care team.

Pay: $33.00-$36.00/HR

Job Requirements:

Required Qualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.   • Graduate of a school of professional nursing. Current Michigan Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.
  • Unencumbered license as a Registered Nurse (RN) in the State of Michigan required.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Ability to read, speak and write fluently in English.
  • Broad knowledge base of physical, mental, and social needs of the frail elderly and their families
  • Skilled in physical assessment and triaging. Ability to react calmly and effectively in emergency situations
  • Strong knowledge of nursing standards and best practices
  • Knowledge of medical equipment and instruments
  • Strong interpersonal communication and phone skills. Ability to prepare written assessments and correspondence. Ability to read and interpret rules and regulations.
  • Willingness to provide on-call coverage within the nursing rotation
  • Ability to foster collaborative working relationships. Ability to supervise, direct, coach, and develop direct reports and larger team. Demonstrated ability to work with a diverse team and flourish in a flexible work environment
  • Strong interpersonal communication and phone skills. Ability to prepare written assessments and correspondence. Ability to read and interpret rules and regulations.

  • Knowledge about the PACE model and philosophy of care
Essentials:
  • Functions as a member of the Care Team. Maintains regular attendance at, and participants in Care Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.
  • In coordination with the Care Manager and other nursing staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team
  • Conducts a comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts nursing re-assessment semi-annually and as needed.
  • Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant's condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.
  • Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.

  • Receives participant complaints, triages, and responds to participant needs.

  • Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.

  • Working with the Care Manager, performs care coordination with outside providers and facilities including hospitals, nursing homes, assisted living facilities, and physician/provider offices. May accompany participants to specialist appointments or make hospital or other facility visits when necessary for coordination of participant care.

  • Provides treatment instructions and nursing education to participants and family.

  • Assists, as directed, in the maintenance of the clinic and exam rooms including stocking of supplies and materials and cleanliness.

  • Maintains accountability for high customer service standards and monitors, evaluates, and recommends opportunities for improved participant and family/caregiver satisfaction.

  • Manages and reviews participant's records, charts, pertinent information and staff's documentation. Documents as needed in medical records according to Senior Community Care standards.

  • Maintains the stability and reputation of SCCOM by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual requirements.

  • Establishes and maintains a productive working relationship with relevant contracted providers.

  • Is responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.

  • Follows and ensures that clinical team follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines.

  • Educates staff in regards to care and treatment for the elderly as needed.

  • Acts within the scope of his or her authority to practice.

  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.

  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.

  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

  • Performs other duties as required or requested.
