GENERALSUMMARY/ OVERVIEW STATEMENT:

The Outpatient Pharmacist reports to the pharmacy manager forBoston, pharmacy coordinator for Revere and Waltham. Pharmacists are expected tobe trained and cover shifts in all 3 pharmacies as needed. Pharmacists areresponsible for supervising technicians and pharmacy interns. Pharmacists areexpected to provide the highest level of pharmaceutical care to patients of MGHthrough excellent pharmacy stewardship, reliable and efficient systems andoperations, and through collegial relationships and teamwork. Assures pharmacyis an integral part of the health-care delivery system and facilitatesenhancement and expansion of pharmacy services/programs.

PRINCIPALDUTIES AND RESPONSIBILITIES:

ThePharmacist collaborates with and serves as a resource for nurses, physicians,and other clinicians who participate in the management of patients receiving careat MGH Outpatient Pharmacy, MGH, and its affiliated Health Centers. He/she is responsible for coordinating and delivering pharmacy services incompliance with all applicable policies, regulatory agency requirements andstate and federal laws.

Job-SpecificResponsibilities:

  • Works with the pharmacymanager/coordinator to meet area specific goals and goals of the department
  • Establishes collaborative pharmacist-patientrelationships
  • Ensures that all applicable Federal and Stateregulations are followed
  • Participates in interviews for hiring personneland interns
  • Preceptor for pharmacy interns
  • Participates in training and development forother pharmacists, technicians and interns
  • Assures safe and accurate dispensing ofmedications
  • Provides accurate verification of medications
  • Performs effective drug utilization review
  • Interprets, evaluates, and accurately processesprescriptions
  • Identifies, mitigates, prevents, and documentsproblems with prescriptions
  • Provides counseling to patients on the properuse of their medications, possible side effects and the role of prescribedmedications in the overall therapeutic care plan
  • Oversight of the prescription processing workflowincluding labeling and proper and safe storage of drugs
  • Manages all queues in the pharmacy software system
  • Monitors service performance metrics
  • Accurately dispenses controlled substances inaccordance with Massachusetts and Federal laws
  • Participates in perpetual inventory ofcontrolled substances
  • Participates in biennial controlled substanceinventories
  • Participates in semi-annual and periodicinventory of all drugs
  • Assists in daily cycle counts
  • Maintains records in accordance withMassachusetts and Federal laws
  • Assists staff with career ladder and staffdevelopment.
  • Adheres to pharmacy policies and procedures
  • Assists in developing new policies andprocedures
  • Coordinates and provides support to clinicalstudent programs.
  • Assists in the return of outdated medicationsto the reverse distributer
  • Ensures the timely processing of return tostock medications
  • Participates in Meds to Beds program
  • Manages patient's adherence through Time myMeds
  • Assists providers with prior-authorization, useof Cover My Meds if applicable
  • Participates in direct patient care.
  • Provides Medication Therapy Management
  • Participates in naloxone training anddispensing
  • Provides drug and drug related information tohealth care practitioners
  • Maintains and manages pharmacy system andautomation.
  • Represents the pharmacy department in multidisciplinarycommittees as needed.
  • Submits timely safety reports and providesfollow up as needed
  • Participates in ordering of supplies through thewholesaler, EBuy, standard register, Staples and other vendors as needed.
  • Utilizes the MassPAT prescription monitoringprogram for appropriate controlled substance monitoring
  • Troubleshoots issues that may occur with the automateddispensing robot
  • Escalates issues tosupervisors as needed
  • Participates in RiskEvaluation and Mitigation Strategy (REMS) compliance and monitoring.
  • Ability to make appropriate decisions, developand complete follow-up actions
  • Provides immunizations to patients
  • Patient-focused interpersonal and communicationskills.
  • Provides excellent customer service to allpatients and staff
  • Respond to patients, co-workers, medical staffand other health care providers in a positive, professional and courteousmanner
  • Ability to provide compassionate care to patients
  • Plays a pivotal role in the MGH culture ofproviding the right care at the right time in the right place
  • Participates in quality improvement and errorprevention strategies


Qualifications
SKILLS/ABILITIES/COMPETENCIESREQUIRED:

  • Knowledge of contemporary outpatient pharmacysystems
  • Knowledge of relevant Federal and State lawsand regulations
  • General clinical knowledge and expertise intraditional therapeutics and patient centered care
  • Excellent comprehension of written policies andprocedures

LICENSES,CERTIFICATIONS, and/or REGISTRATIONS (if applicable):

Validlicense to practice as a pharmacist in MA

ObtainsCE credits needed for license renewal as required

EDUCATION:

BSdegree in Pharmacy or PharmD degree required

EXPERIENCE:

  • 3 years' experience


SUPERVISORYRESPONSIBILITY (if applicable):

  • Pharmacy technicians-10 FTE
  • Pharmacy Interns-2 FTE


FISCALRESPONSIBILITY (if applicable):

  • Pharmacy inventory.
  • Pharmacy reimbursement


WORKINGCONDITIONS:

  • Standing, walking, able to lift 25 pounds
