ABOUT THE JOB
The CCBHC is comprised of several community-based programs providing direct care to over 2,500 clients annually, both on site and in the community. Our staff use a culturally competent, person-centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.

The Peer Specialist will provide inpatient-to-community transition care, in-home crisis stabilization/treatment services/supports. The peer will support client in linkage to care and engagement with identified onsite providers and community providers and ensure that client support networks are sufficient to meet their needs.

Responsibilities include:

• Providing emotional and practical support to participants as they engage in behavioral health treatment
• Advocating with and for program participants to achieve personal goals and address barriers such as fear, stigma and communication disparities with community providers and supports.
• Attending and participating in team meetings and supervisory sessions
• Monitoring and record participant's progress and modifying plan according to needs and preference
• Complying with all required in-service training and staff development
• Escorting consumers to scheduled appointments as needed
• Demonstrating excellent organizational and time management

TO QUALIFY
The Jewish Board is currently looking for talented Peer Specialists of all cultures, religions, races, and gender expressions with the following qualifications:

• High School Diploma or equivalent is required
• Relevant lived experience
• Have at least one year experience in direct services working with persons who have been given a psychiatric diagnosis and/or trauma survivors
• At least two years of active participation in mental health self-help activities, peer support or peer advocacy programs, or participant-run organizations or similar experiences or programs.
• Familiarity with NYS Entitlements, Housing and Mental Health Service Delivery System.
• Prior experience with electronic health records and Microsoft Office
• New York Peer Specialist Certification through OMH is a plus
• Must be able to travel around New York City
