STARTING SALARY: $24.04/hr

POSITION SUMMARY:

The peer health navigator works within the Court-Ordered Assertive Community Treatment (CO-ACT) team in the Criminal Justice Services Department. They provide services to assigned individuals to link them with community resources and help them negotiate and navigate health information and systems from a peer support lens, to support of each person's overall well- being and success in the CO-ACT program. This position includes working on a multidisciplinary team, community education and outreach, and crisis intervention when needed.

This ideal candidate is an individual with heart and passion for providing education and peer support to individuals in the criminal justice system with mental health and substance use concerns. The peer health navigator will also serve as an advocate and navigator for individuals with the goal of increasing behavioral health knowledge, medication compliance, and promoting increased access to and engagement in services for CO-ACT participants.

Job Requirements:

  • Orient persons we serve to program outline, requirements, and available services.
  • Report clinical concerns and high-risk persons and safety issues as they occur to other program staff and supervisor at staff report times; follow clinical direction.
  • Work closely and communicate directly with individuals' medical and behavioral health providers to help increase access to needed services and navigate our systems
  • Proficiency in written and spoken English skills.
  • Assist in developing and facilitating Skills Groups.
  • Assist persons in obtaining services that suit the individual's recovery needs including community and natural supports in addition to organizational supports.
  • Attend program team meetings and supervision as scheduled.
  • Attend training as identified by Program Manager.
  • Providing support to the clinical team for certification paperwork.
  • Responsible for to provide personalized guidance to assigned individuals as they move through the healthcare system.
  • Fully disclose relevant training, experience, and required credentials, to help individuals understand the scope of services.
  • Must have a clear understanding of HIPAA guidelines, confidentiality, personal boundaries and be self-assured in a variety of situations.
  • Consistent with grant priorities as informed by Program Manager.
  • Maintain a trauma informed environment of wellbeing.
  • Perform other duties as assigned.

EDUCATION: High school diploma or equivalent work experience. Bachelor's Degree in psychology, social work, or other related human services degree preferred.

CERTIFICATION: CAT/CAS certification preferred.

Experience:
  • Personal, lived experience working toward recovery in mental health or substance use.
  • Preferred minimum one-year experience in mental health, substance use or human services.
  • Preferred experience working with unhoused individuals or/and criminal justice involved individuals.

SKILLS AND COMPETENCIES:
  • Knowledge or ability to learn and practice trauma informed principles and practices.
  • Demonstrates the ability to work with people from all walks of life.
  • Knowledge of health care/behavioral healthcare systems and community resources
  • Demonstrates the ability to work effectively with community members and other community agencies and partners.
  • Ability to work as a team member in a multidisciplinary setting.
  • Ability to give, receive and analyze information, formulate work plans, maintain confidentiality, prepare written materials, and articulate goals and action plans.
  • Solid interpersonal skills.
  • Ability to operate a computer and required software programs.
  • Strong time management skills.

SUPERVISORY RESPONSIBILITIES: None

PHYSICAL REQIUREMENTS: Ability to sit, stand, bend and reach, drive a vehicle, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasionally lifting to 40 lbs.

WORKING CONDITIONS: May require occasional evening, or weekend hours depending on program and consumer needs. Conditions meet the requirements of applicable federal, state, and organizational background checks.
