Join our team of competent and compassionate professionals as an Outreach Counselor in the Behavioral Health Justice Involvement program at Behavioral Health Network!

Your experience, knowledge and great skills will support the BHJI program with the following:

  • Providing in-person, telephonic, and Telehealth care management supports to individuals served based on the collaborative spirit of the integrated health model that promotes improved health, independence, and self-sufficiency.
  • Meeting individuals admitted to 24-hour facilities (e.g., hospitals and substance use disorder treatment facilities) in-person or via Telehealth and maintains regular contact with them during their stay to assist with discharge planning and re-engagement with outpatient providers, community services, and social supports.
  • Acting as primary "point of contact" for all individuals served on caseload, ensuring their needs are met.
  • Completing Bio-Psycho-Social Assessment, Individualized Action Plan, Crisis Plan, and Discharge Plan in collaboration with providers and supports.
  • Including all appropriate people in the individual's care including primary care physician, nurses, mental health and substance use providers, and natural supports as identified by the member, with their written consent.
  • Demonstrating working knowledge of community services and how to help individuals access them including health insurance, food and nutritional assistance, housing placement and stabilization, financial assistance, utility assistance, educational/vocational supports and legal assistance.
  • Meeting required productivity expectations for billable services.

Who are we?

The Community Support Program (CSP) is a short-term program designed to help adults with mental health or substance use challenges to live independently in the community. BHN CSP serves adults who have recently been in or who are at risk for psychiatric hospitalization or admission to detox, providing access to community resources such as transportation, housing stability, and day programs along with others.

What are we looking for in an Outreach Counselor?

Qualifications/Requirements
  • 1-2 years of work experience in a related behavioral health, substance use, and/or related human services field is required.
  • High School Diploma or GED Required
  • Community Health Worker Certification and/or bachelor's degree is preferred.
  • Bilingual, Spanish and English Speaking is preferred.
  • Knowledge of mental health and substance use disorders, experience with justice involved populations and justice entities, care management services and how to access community resources.
  • Valid driver's license required.
  • Vehicle for use during all working hours should be registered and inspected in accordance w 540 CMR 4.00.

**NOTICE**

In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work.

The same guidelines noted above apply during flu season (defined as December 1st - through end of flu season as announced by the CDC) for BHN's Mandatory Influenza Vaccine policy.

Additionally, BHN is a Tobacco Free workplace. All Staff, interns, temps, residents and contractors agree not to smell of or use Tobacco or ENDS products while on paid work time or within sight of any BHN facility.
