Location : This position will work a hybrid model (remote and office). Ideal candidates will live in the state of Indiana and reside within 50 miles the PulsePoint on 220 Virgina Ave.

Responsible for coordinating services with Adult Protective Services (APS), Long-Term Care Ombudsman, Member Support Services, and the Department of Health (DOH).

Primary duties may include, but are not limited to:

  • Serve as the single point of contact and responsible for coordination with APS, the justice system, Member Support services contractor and the Long-Term Care (LTC) Ombudsman.
  • Tracks and coordinates LTC Ombudsman referrals and for Indiana PathWays for Aging members and identifies members who may be at risk of abuse, self-neglect and exploitation and in need of adult protective services (APS_ or the services of the State LTC Ombudsman or DOH.
  • Develops and executes statewide ombudsman strategy for the Indiana PathWays for Aging population.
  • Improves understanding of local advocacy organizations, philosophies, stakeholders, legislators and providers that interact with or influence members and policy.
  • Serves as an informal information resource, communication channel, complaint handler, facilitator, consultant, and practitioner for dispute resolution.
  • Supports coordination of members physical, behavioral, and long-term care needs, including coordination and participation in care planning with the LTC Ombudsman as appropriate.
  • Seeks fair/equitable solutions to member/provider problems.
  • •Provides support during facility closures or transitions and collaborates with the LTC Ombudsman to ensure all member discharge rights are upheld.
  • Recommends dispute resolution processes to address and manage conflicts; recommends policy and procedural changes.
  • Brings issues to health plan executive leadership to address care delivery improvement efforts and promote effective relationships/communication between members and providers.
Minimum Requirements:
  • Requires a BA/BS degree and a minimum of 7 years of related experience; or any combination of education and experience, which would provide an equivalent background.
  • Proven track record of developing programs and services for Children at risk or in Foster Care.
