Location: Eugene, OR
Description: Our client is currently seeking a Nurse Practitioner to work in Eugene, OR!!

Location: Eugene, OR
Hours: Monday - Friday 8:00 AM - 5:00PM

Type of Engagement: 7-12 month Contract

Responsibilities:

• Conduct thorough in-person patient assessments to identify and address health concerns.
• Effectively manage acute and chronic medical and behavioral conditions, collaborating with specialists as needed.
• Maintain accurate and complete patient records, including diagnoses using ICD-10 coding standards.
• Foster open communication and collaboration with team members like physicians, nurses, social workers, and therapists to develop and implement comprehensive care plans.
• Guide patients and families through advanced illness and end-of-life care discussions with sensitivity and compassion.
• Provide patients and caregivers with education and counseling to support informed healthcare decisions and improved self-management.

Qualifications & Requirements:

• An active and unrestricted license to practice as a Nurse Practitioner (NP) in the state you reside in, or the ability to obtain licensure by the start date.
• A Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited program.
• Board certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) in one of the following areas:
• Family Nurse Practitioner (FNP)
• Adult Nurse Practitioner (ANP)
• Gerontology Nurse Practitioner (GNP)
• Adult-Gerontology Acute Care Nurse Practitioner (AGACNP)

A plus if you have:

• One or more years of clinical experience, ideally in a long-term care setting.
• Proven experience in addressing the medical needs of patients with complex behavioral, social, and/or functional challenges.
• A strong understanding of geriatrics, chronic illnesses, and advanced illness care.
• Experience and comfort facilitating sensitive discussions about advanced illness and end-of-life planning.
• Proficiency in computer skills, including the ability to utilize electronic medical records (EMR) for documentation.
