BASIC FUNCTION AND RESPONSIBILITIES:

  • The Nurse Clinician - Specialist (NC-S) will coordinator patient care for the transplant patient population across the continuum as pertains to transplantation and be the primary resource for the patient though out the transplant process. The NC-S will use a multidisciplinary team approach for end stage organ disease patients, resulting in a comprehensive individualized plan care to guide the patient through the multiple phases of transplant care; leading to enhanced patient satisfaction, improved quality of care and the appropriate utilization of resources. The Nurse Coordinator - Specialist is responsible for maintaining compliance with multiple Regulatory Agencies including CMS, OPTN/UNOS, JC.
Characeristic Duties and responsibilities

  • Gather pertinent medical information prior to clinic visits to anticipate patient needs for ongoing care and management of transplant candidates and recipients. Assimilates the data in a cogent manner and integrates this data into the EMR for the patient so it is available for all providers.
  • Through the use of data driven protocols, the coordinator monitors and documents in the EMR, pertinent patient care information across the continuum of the solid organ transplant process.
  • Provides complete and timely communication with external providers to assist the patient through their various transitions of care.
  • Assists with communication to insurance payer, including utilization management functions of prior approval and explanation of medical necessity.
  • Facilitate adequacy of insurance coverage for patient through coordination of information with Financial Counselors and billing personnel.
  • Obtains insurance approval for care at UIHC; which includes agreements for special drugs, interventional procedures and/or diagnostic testing.
  • Fully participates in the systematic review of protocols as they pertain to the abdominal solid organ transplant population.
  • Provides patient education that is in depth and meets not only regulatory agency standards but patient unique needs.
  • Is the primary resource for the patient and their family throughout the transplant process.
  • Attends and participates in multidisciplinary rounds, care conferences and educational offerings in the department
  • Assists in the mentoring of new staff in outpatient solid organ transplant units. Participates in educational offerings for nursing staff in in-patient units.
  • Participates in process improvements efforts and projects within the department as part of the overall QAPI plan.
  • Assists the social worker to ensure adequate medical information is provided for entitlement/categorical program applications to be completed.
  • Provides off shift on - call coverage for the Organ Transplant Center.
  • Reviews outside medical records received in preparation of new patient referral scheduling. Reviews documents with focus on items that indicates referral may not meet program criteria for candidate consideration or concern.
  • Makes first contact with new patient referred for transplant.
  • Provides an over view of the transplant process, confirms pertinent information found in outside records. Informs patient of any additional data that if available will help expedite the evaluation process.
  • Places orders, using organ/disease specific transplant protocols for physician and ancillary appointments, laboratory and diagnostic testing required for complete transplant evaluation.
  • Provides telehealth support during the evaluation process.
  • Meets the patient in the ambulatory clinic to address concerns and provide additional education regarding the evaluation process.
  • Prepares and presents a comprehensive overview of data gathered throughout the evaluation for the multidisciplinary care conference.
  • Schedules the patient for any follow up testing after the care conference.
  • Communicates the multidisciplinary team decision regarding eligibility for proceeding to transplant.
  • Places the patient on the United Network for Organ Sharing (UNOS) waitlist.
  • Follows all regulatory guidelines for transcription of data into the secure web site for UNOS.
  • Is the primary telehealth resource for the patient and family while the patient waits for an organ offer.
  • Monitors health of the patient while on the waitlist.
  • Maintains communication with outside providers and insurance carriers regarding patient status on the waitlist.
  • Schedules routine on site assessments of the patient to assure ongoing suitability for transplant.
  • Works with on call team at the time an organ offer is made.
  • Coordinates communication with patient and family.
  • Coordinates the hospital admission process.
  • Communicates appropriate details of the organ offer, patient condition, and transplant surgery to ancillary personnel.
  • Participates in systemic review and updates of patient and family and living donor educational tools.

Percent of Time: 100%.

Schedule: Days, Monday - Friday 8AM - 4:30PM, will take call after a year

Required Qualifications
  • A Baccalaureate degree in Nursing is required.
  • Current license to practice nursing in Iowa is required by date of hire.
  • Current experience (within the last 18 months) with Solid Organ Transplant or end organ failure is required.
  • Demonstrated experience in patient education is required.
  • Proficiency in computer applications including Microsoft Excel, Word and PowerPoint is required.
  • Excellent written and verbal communication skills are required.
  • Demonstrated excellent organizational skills, critical thinking, and problem solving skills are required.
  • Demonstrated expertise with adult patient/family discharge planning process is required.
  • Experience working with an interdisciplinary team is required.
Desired Qualifications
  • Master's degree in nursing is desired.
  • Previous care coordination is desired.
