Provides direct nursing care, counseling and instruction to patients in a health care clinic. Interviews patients and triages telephone calls to determine and assess patients' health care needs; develops plan of care or refers, as necessary. Serves as an advocate and provides instruction and counseling to patients and their families. Documents nursing actions in patients' records to provide accurate medical records for future reference and accountability. Participates in interdisciplinary planning for meeting patient care and health service objectives. Performs necessary related activities to maintain clinic operations. Coordinates services for patients utilizing other human service-related agencies and resources; follows up on health care of patients. Other duties as assigned.
Unit Specific Education/Experience/SkillsKnowledge equivalent to that which normally would be acquired by completing a four year college degree program in Nursing; one to three years of related and progressively more responsible or expansive work experience in Nursing; or an equivalent combination of education and experience. The position also requires possession of a current license to practice as a Registered Nurse in the State of Michigan. May require a Bachelor's degree in Nursing and/or work experience in a particular area of nursing or health care.

Summary of Physical DemandsThe job requires significant physical effort and involves lifting and maneuvering patients, extensive walking and standing and fine manual dexterity 26-75% of the time; visual and aural acuity necessary to monitor patient care.
Summary of Health RisksExposure to human blood, serum, and other body fluids; and materials covered under Universal Precautions; TB risk or work within three (3) feet of patients in a health care setting; wear a respirator.
