Description:

The Nephrology Advanced Nurse Practitioner or Physician Assistant provides advanced nursing care in both the inpatient and outpatient dialysis setting.

The APN or PA assumes the management of patients, functioning in both an independent and collaborative effort with the physician, case manager, and other members of the health care team.

The APN/PA designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed. The APN/PA facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching. Ensures that care delivered to patients, meets/exceeds standards of care and is provided according to Loyola University Medical Center's mission, strategic plans and all internal/external regulations, policies and procedures. Practices within the Illinois Department of Professional Regulation Nurse Practice Act.

Supports the established care models of Loyola University Medical Center. Functions independently and collaboratively with the physician to provide continuity of health care services for individuals, families or groups. Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment. Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution.
