Description:
The Social Worker will work in collaboration with case managers and other healthcare professionals to enhance cost-effective use of resources by identifying and initiating complex discharge planning to accomplish the timely transition of patients to post-hospital settings. The social worker must have in-depth knowledge of system and community resources and referral agencies. They are responsible for communicating with external agencies to provide referral information for a smooth transition from hospital to the post-hospital setting. This includes communicating with other health care agencies and payer sources. The social worker will also provide assistance to patients, families and others with a wide variety of needs to include, but not limited to, discharge planning, financial concerns and/or emotional problems related to the current acute illness. Provides supportive counseling to patients regarding adjustments to illness as well as other psychosocial needs as indicated.
Position Requirements:

Certification/Registration: Current MSW degree required.

Education: Must be a graduate of a Master of Social Worker Program.

Experience: Must have a minimum of 3 years experience in social work, including a minimum of 2 years in hospital discharge planning

Special Qualifications: Excellent communication, interpersonal, and critical thinking skills.

Full Time, will work 24 hours but paid for 36 hours. Saturday and Sunday, 8:00am - 8:00pm
