Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Life Coach will work closely with young people at the highest risk for perpetrating or being the victims of violence. The Life Coach position is a multi- faceted role, serving as a critical anchor in violence reduction efforts. As a cornerstone to the Eskenazi Health violence prevention reduction programs. The Life Coach works alongside young adults 18-35; engaged in street violence as they receive a host of supportive services that include Cognitive Behavioral Therapy counseling (CBT), health education, intensive case management, educational attainment, and job readiness training. The Life Coach is responsible for critical data capture, tracking individual progress from assessment through each phase of support, and the programming that follows.

Essential Functions and Responsibilities

• Provide support and accountability to a caseload of 12-20 individuals in their reduction of risk increase of safety through a coordinated, and documented process of support.
• Complete and coordinate intake and assessment process
• Conduct and document weekly Life Coaching sessions with every participant on caseload, setting and monitoring achievement goals, case management and program progression
• Document significant contacts, events and progress of each individual
• Create coordinated care, addressing gaps in information and services for each participant through regular communication with community service providers, family members, and other identified supports, including transitional employers.
• Enforce all agency policies, including but not limited to Eskenazi Health COVID safety plan, Workplace safety policy, AIDET, attendance policy, and engagement in community activities.
• Assist with the facilitation of CBT skills groups and other group facilitation.

Job Requirements

• High School diploma or GED required; Associate's Degree in Social Work, Psychology, Counseling, or related field of study preferred, or equivalent years of experience as a community outreach worker, mentor, job coach may be accepted in lieu of education
• Experienced mentor with some career coaching, training and development, and/or workforce development with a focus on young adult population.
• Experience working high-risk population, in the field of violence prevention
• Case management skills/experience
• Demonstrated track-record of accountability
• Valid Indiana Driver's License. Work is performed in the office and in the field.
• Passion about the reduction of Gun Violence
• A background and understanding of the population served, having a similar lived experience as the clients to be served

Knowledge, Skills & Abilities

• Excellent communication, coordinating, planning and presentation skills
• Knowledge of computers, including Word, Excel, and Internet
• Knowledge of the psychology of poverty, abuse, neglect, substance abuse and family dynamics and their effects on family and children
• Knowledge of working with individuals who have experienced or suffered from trauma, PTSD and or anxiety
• Knowledge of education, employment, healthcare, and other community resources available to target population
• Excellent relationship building skills as well as customer service skills
• Attention to detail, strong organizational, time management and problem-solving skills
• Prioritizes tasks and works effectively with complex tasks under time pressures
• Communicates in a concise and easily understandable manner with professionals, paraprofessionals, and the target population
• Respect for and sensitivity to the needs, rights and cultural, racial and ethnic differences
• Ability to be nonjudgmental and to mentor and encourage strength based approach to patient/client services
• Practice compassion, empathy and insight
• Acts as a patient advocate
• Knowledge and skills necessary to provide care appropriate to the age of patients served. Must demonstrate knowledge of growth and development principles over the life span and possess the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age-specific needs. Ability to use age appropriate communication and skills detailed in the department/area/unit job specific/age specific competencies
• Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, initiative; be flexible and dependable, and demonstrate initiative
• Ability to perform duties independently under the direction of specified supervisor
• Ability to recognize own limitations and seek assistance from appropriate resources as needed
• Willingness to learn and be trained on effective violence reduction strategies as well as professional development
• Ability to work some weekends and evening hours as needed.
• Ability to effectively deescalate conflicts with participants.
• Ability to challenge antisocial behavior and consistently hold participants to high standards
• Ability to develop trustworthy partnering relationships with their clients, helping them to achieve their individual goals
• Attend local community workshops and trainings
• Willing and able to navigate challenging neighborhoods, people, and families
• Ability to evaluate and assess the needs of participants, and adapt processes according to the needs of the patients and families
• Ability to understand the concept of violence as a public health issue
• Ability to recognize own abilities and limitations in provision of services to participants
• Ability to meet with patients and families and to develop a trust level necessary to provide services and to encourage positive outcomes
