Job Title: Hospice RN Case Manager

Duties & Responsibilities: Hospice RN Case Manager sought for 12 week assignment with the potential to extend. Schedule would be M-F 5x8 day shifts.

Minimum Requirements:

• IN or Compact RN license
• 1+ year hospice in home setting experience within last 3 years
• HCHB experience preferred but not required
