GENERAL SUMMARY/OVERVIEW STATEMENT:

The Transgender Health Program (THP) is currently seeking a full-time, Family Nurse Practitioner to join in our mission and act as a physician extender in helping to provider primary and specialty care to pediatric and adult patients, as well as support and engage in the various initiatives aimed at affirming the identities of our patients.

Under the supervision and direction of the Medical Director of the Transgender Health Program the Family Medicine Nurse Practitioner is responsible for assuring competent, compassionate, gender- affirming care for patients and their families. The FNP is the clinician in the office primarily responsible for providing acute care for the patients who receive their primary care at the MGH THP. The FNP will also provide follow-up care for patients who receive primary or specialty care at the THP in collaboration with the patient's primary physician provider. The FNP will be expected to collaborate and coordinate care with the physician primarily caring for the patient and will not carry their own patient panel. They will be expected to follow up on the management of gender affirming hormone therapy and pubertal blockade and may initiate this therapy with guidance from physicians. She/He/They collaborate with the primary care physicians, members of the patient care team, support staff and Social Workers to achieve optimal patient outcomes.

Individuals with experience in Psychiatry work and PMHNP programs will be considered.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical Care:

  • Primarily provides acute or urgent care for patients who receive their primary care at the MGH THP
  • Provides clinical visits virtually or in-person
  • In collaboration with the primary physician for the patient, provides both acute and continuity care which includes:
    • Developing and implementing disease-specific patient management plans
    • Assessing, instructing and counseling patients regarding compliance with treatments
    • Ordering, interpreting, and evaluating diagnostic tests to identify and assess patients' clinical problems and health care needs
    • Prescribing or recommending treatment
    • Maintaining accurate and timely notes in the electronic medical record
    • Billing appropriately for the visits conducted
  • Collaborates with outside facilities and clinical staff for certain patients to provide general information to other caregivers including outside hospitals, PCP's, VA and private help
  • If qualified, may provide short-term psychopharmacology and short-term therapy to patients needing interim management.
  • Performs other duties as assigned
Other:
  • Support MGH THP in facilitating training sessions at MGH and MGB affiliate networks
  • Provides support and assistance with challenging patient/physician situations
  • Actively participates in network development initiatives and communicate initiatives back to access team for relationship building within MGH affiliate networks
  • Participate as requested in MGH and MGB quality initiatives
The incumbent will have at a minimum 80% clinical time providing direct clinical care and/or supporting clinical care for patients. The remaining 20% will consist of administrative duties, educational engagements, and other programmatic support efforts.

Qualifications
SKILLS/ABILITIES/COMPETENCIES REQUIRED
  • Spanish and/or Portuguese speaking preferred
  • Computer literacy in Word, Power Point, and Excel preferred
  • Able to utilize technologies such as Epic EMR, Zoom, and other resources to make sure patient care is well documented; training in Epic EMR will be provided for those who have not used this interface previously
  • Possesses excellent assessment, critical thinking, judgment and interpersonal skills
  • Prioritizes and manages time efficiently
  • Works effectively in an independent manner and within the scope of professional practice guidelines
  • Collaborates and coordinates care with physicians
  • Works effectively within a multidisciplinary team and practice setting
  • Receives and incorporates feedback on practice well
  • Deep understanding of the transgender, non-binary, gender-expansive community, and their healthcare needs
QUALIFICATIONS
  • Master's degree in Nursing required.
  • License Nurse Practitioner License required., and Certificate 1 Basic Life Support required.
  • MA Board authorization to practice as an APRN NRSG
  • Valid prescriptive authority from both the MA DPH-DCP and Federal DEA
  • American Heart Association - Basic Life Support Certificate
EXPERIENCE:

Minimum two (2) years of experience in nursing care - either through direct clinical care or education, working with vulnerable populations.

WORKING CONDITIONS:

• Office setting in an outpatient medical clinic

• Some remote work may be incorporated in compliance with infection control/COVID-19 efforts

SUPERVISORY RESPONSIBILITY:

• None

FISCAL RESPONSIBILITY:

• Prudent use of hospital resources
