Job Summary:
The Counselor/Case Manager is primarily responsible for providing case management and supportive counseling, individually and in group sessions, developing and implementing therapeutic treatment plans in the framework of recovery to the clients of the Caspar Men's Residential program with (SUD) Substance Use Disorder and other varying levels of impairment while, supporting the mission of Bay Cove Human Services. This is a non-exempt position.

Essential Job Duties and Responsibilities:

  • Ensures the completion of the psychosocial, admission MIS form, release of information forms and treatment plan within the first 24-48 hours in EHANA after client admission, and determines with client (and co-workers when appropriate) the individual treatment and aftercare plans.
  • Provide individual counseling to all clients regarding substance use.
  • Provide/lead group counseling to all clients regarding substance use and other topics and submit group notes in EHANA. Group topics can include, but are not limited to educational skills, life skills, coping skills and relapse prevention.
  • Provide case management and submits weekly progress notes in EHANA.
  • Promotes ongoing participation of the client with regards to decisions involving plans, goals and progress.
  • Assists clients to arrange essential appointments and accompany clients (when necessary and appropriate).
  • Assists in TEAM effort when handling emergencies and provide crisis intervention when necessary.
  • Coordinates and engages in treatment services and care consultation regarding specific clients with other staff, relatives and appropriate representatives of external treatment providers/agencies, including social service agencies, housing authorities, health care providers and vocational programming.
  • Make appropriate referrals and complete follow-up referrals to insure proper aftercare.
  • Completes discharge summary and submits in EHANA in a timely manner.
  • Maintain clients records and programmatic documentation in EHANA and insures their confidentiality in a timely manner.
  • Complete and maintain clients monthly medications record sheets.
  • Serves as an advocate and resource, providing the client with community support information.
  • Attend and participate in all multidisciplinary meetings, clinical meetings, staff meetings and supervision as required.
  • Implement and comply with the policies and procedures of the program and Bay Cove Human Services.
  • Complete all required trainings in a timely manner.
  • Other duties as assigned by Program Director
Supervisory Responsibilities: None

Reports to: Clinical Director

Requirements for the position:
  • The person in this position must have a clear commitment to the population we serve, and be able to work as part of a TEAM.
  • BA/BS preferred (degree requirement waived with additional experience)
  • Two years case management and/or direct care experience in areas such as homeless services/housing search, substance use treatment, mental health, and/or harm reduction services.
  • Proficiency in Microsoft Office (Windows) and Google Apps.
  • Demonstrated professional communication and organizational skills
  • A valid Mass driver's license in good standing
  • Knowledge and/or experience regarding the CSPECH Program preferred
  • Bilingual Spanish/English speaker preferred
  • A COVID-19 vaccination is a requirement of the position. One COVID-19 shot is acceptable, contingent on the individual receiving the second shot within the allotted time frame.
Skills Required:

  • Knowledge and familiarity of evidence based practices and trauma informed treatments/approaches.
  • Good written and verbal skills, computer skills, record keeping and organizational skills.
  • Ability to follow and implement the policies and procedures of the program.
  • Ability to work independently and responsible.
  • Ability to accept supervision and be part of a TEAM.

  • Ability to work effectively independently and within an interdepartmental team.
  • Sensitivity to the needs of adults with mental illness, substance use and/or developmental disabilities.
  • Commitment to improving the quality of services available to people with disabilities
