Affiliated Pediatric Practices

Affiliated Pediatric Practices (APP) is a group of 17 independently-owned, Massachusetts-based community pediatric practices. Our goal is to offer excellence in healthcare to our pediatric patients and families by combining the highest-quality care with APP's unsurpassed, outstanding efforts and dedication toward providing exceptional patient experiences. Location

This position is a hybrid role, with 2 days remote and 3 days onsite at APP Central in Dedham MA.

General Overview

Under the general direction of the Community Health Team Lead, the Community Resource Specialist (CRS) is an integral part of the APP Community Health Team in providing support to primary care practices. The CRS is responsible for managing requests from primary care providers for social determinants of health (SDoH) and other community resources on behalf of their pediatric patients. The Resource Specialist researches and identifies a customized list of resources to meet the needs of the individual patient and conducts outreach to help patients navigate and connect with internal and external resources. The position will be involved in proactive screening to identify gaps in patient needs and working with the care team to help families connect to resources to close those gaps. This position requires a high degree of flexibility, independence, and willingness to participate in multiple activities and provide support to all members of the care team. The position requires strong communication skills, and the ability to converse comfortably with patients and their caregivers, practice and program staff.

Qualifications
Qualifications

  • Bachelor's Degree in social work, psychology, public health (or related field) preferred.
  • 2 years' work experience, preferably in the health care field
