FUNCTION:

The Community Living Counselor (CLC) is responsible for the provision of direct support services/Psychiatric Rehabilitation Program (PRP) services for eligible residents to promote residents' ability to participate in appropriate community activities, and contribute to the development and utilization of skills necessary for independent living. In addition, the CLC, like all staff onsite, is responsible for promoting and maintaining the security and safety of residents, staff, visitors, property and all other assets.

The CLC will work closely with the case management staff in the assessment of residents' needs, and the development, implementation, and monitoring of the resident's individualized rehabilitation or program plan (IRP/IPP). He/She is also responsible for providing crisis intervention.

All delegated actions taken by this employee will be carried out in accordance with all VOAC policies, procedures and guidelines. Driving, including transportation of clients in accordance with need and program requirements, is a responsibility of this position.

PRINCIPLE ACTIVITIES:

  1. Maintain a working knowledge of and compliance with VOAC.'s policies and procedures, and program operations policies and procedures.
  2. Ensure resident compliance with VOAC and facility policies and procedures.
  3. Ensure resident compliance with their IRP and/or IPP.
  4. Participate in the assessment of resident's service needs, and in the development, implementation, and monitoring of the IRP and/or IPP.
  5. Provide Community Living Support/PRP services as required in accordance with COMAR and other regulations for Residents and community clients.
  6. Participate in the development or restoration of resident's self care skills (including personal hygiene, grooming, socialization skills, and maintenance of the individual's living environment).
  7. Coordinate and facilitate rehabilitation meetings and/or groups for residents, addressing the residents' rehabilitation needs.
  8. Assist in the supervision of activities, including children's activities, within the facility, ensuring that a safe environment is maintained.
  9. Coordinate activities, including transportation, which promote community integration.
  10. Communicate with Case Management staff concerning resident progress.
  11. Document service delivery and progress information as required. Maintain resident files
  12. Maintain a caseload of twenty (20) clients and bill for all eligible clients
  13. Implement and operate information systems as required, including HMIS, Medicaid billing systems and case management systems.
  14. Attend staff meetings, training, house and program meetings as directed.
  15. Investigate and report all security and hazardous conditions or incidents; follow-up as assigned.
  16. Respond in a timely and appropriate manner to emergency and/or crisis situations according to established guidelines.
  17. Mediate conflict resolutions on premises, and follow appropriate procedures.
  18. Monitor the building entrance, answer telephones, screen visitors, and direct their movement around the facility.
  19. Perform other duties as assigned. PHYSICAL REQUIREMENTS

The physical requirements described below are representative of those that must be met by an employee to successfully perform the essential duties of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties.
  1. The ability to safely operate a motor vehicle to transport oneself, clients and program supplies as necessary;
  2. The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment;
  3. Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment;
  4. Operating office equipment requiring continuous or repetitive hand/arm movements;
  5. The ability to remain in a sitting position for extended periods of time.


EFFECT ON END RESULT:
  1. Support and mental health services will be provided which address the resident's needs.
  2. A safe and secure environment will be maintained for residents and staff.
  3. Residents will participate more independently in activities of community life.
  4. Resident socialization and life skills will be enhanced, which will aid residents in reaching a more realistic degree of self-sufficiency.
  5. Residents will move out of Pratt House into less restrictive settings.
  6. Resident files and information systems will be maintained which are in accordance with VOAC, facility and regulatory guidelines.
  7. Volunteers of America Chesapeake, Inc. policies and procedures will be implemented and programmatic requirements will be met.

REQUIREMENTS:

The successful candidate must possess a Bachelor's Degree in Human Services and have one year of experience working with individuals involved in social, resident and/or rehabilitative services. Bachelor's Degree required, Master's Degree preferred.

Most importantly, the successful candidate must be able to develop effective working relationships with the resident population, be able to relate to individuals making significant life decisions, be supportive of an individual's increasing independence, be able to handle conflicts and assist with resolutions, and be able to handle emergency and or crisis situations. Successful candidates must also be able to provide effective training, prepare written reports, maintain resident documentation and effectively operate computer systems including HMIS and Medicaid/PRP systems.
