Location: BSWH Grapevine Emergency Department

JOB SUMMARY

This Community Health Paramedic has primary responsibility for the care and management of patients enrolled in the mobile integrated health program. This position will be responsible for facilitating the continuity of care by interacting with the patient's physician and alternative healthcare facilities. The position will also be responsible for all aspects of patient care to include, but not limited to assessment, interaction and treatment per the departments protocol, policies and procedures of each patient contact.

ESSENTIAL FUNCTIONS OF THE ROLE

Provides urgent, emergent and non-emergent care based on patient assessment and condition by functioning as a field paramedic performing direct care activities in accordance with offline clinical protocols and safety requirements.

Communicates with multiple agencies to facilitate continuity of care objectives, and collaborates with other departments as it pertains to case coordination and overall mobile integrated healthcare (MIH) program operations.

Prioritizes level of care based on patient, clinician, healthcare system and care plan needs.

Assists in developing processes and education material pertaining to MIH and critical care programs.

Operates in various clinical settings to provided patient assessment and care from non-emergency to long-term patient care and monitoring in a non-traditional setting.

Manages the course of care, care plans and all communications in the emergency medical responder (EMR), as appropriate.

Documents activities completely and thoroughly to ensure appropriate information is available regarding each call.

KEY SUCCESS FACTORS

ACLS and PALS preferred.

Effective written and oral communications. Ability to clearly communicate with other healthcare providers.

Familiar with computer programs such as Excel, Word, PowerPoint, etc. and EMR documentation software used for documentation in the patients record.

Must be team oriented with the ability to work efficiently and effectively with others.

Ability to multi-task and make sound decisions in critical situation.

Ability to troubleshoot and determine resolution.

Ability to effectively delegate workloads.

Bilingual Spanish speaking a plus.

QUALIFICATIONS

- EDUCATION - Associate's or 2 years of work experience above the minimum qualification

- EXPERIENCE - 4 Years of Experience

- CERTIFICATION/LICENSE/REGISTRATION -
      &nbspDrivers License (TXDL)

      &nbspBasic Life Support (BLS): BLS required.

      &nbspEmerg Med Tech - Paramedic (EMT-P), Licensed Paramedic (LICPARA): Must have an EMT-P or be a licensed Paramedic.
