The University of Iowa Health Care, Department of Pharmaceutical Care, is seeking a 75% Clinical Pharmacist for Acute Care - Internal Medicine. The clinical pharmacist is responsible for all aspects of pharmaceutical care for adult patients in acute care areas/units including, but not limited to internal medicine and cardiology. The position will have rotating shifts including weekdays, weekends and evenings.

Clinical pharmacists at UIHC are involved in all aspects of patient care including, but not limited to, therapeutic drug monitoring, admission and discharge medication reconciliation, medication education, and precepting pharmacy learners. In addition to all members of the pharmacy team, they collaborate daily with other healthcare providers including, but not limited to, licensed independent practitioners, medical students and residents, nurses, and social workers and care coordinators.

Duties to Include:

Provides Therapeutic Drug Monitoring (TDM)

  • Evaluate medication orders to ensure accuracy and appropriateness
  • Assure pharmacokinetic assessment of drug dosing on appropriate patients
  • Communicate with prescribers on patient medication needs
  • Identify and evaluate adverse drug reactions on patients
  • Provide drug information to other healthcare professionals
  • Confirm/clarify patient drug allergies noted in medical record when needed
  • Complete medication histories on all patients and reconcile home medications against the admission medications
  • Review and reconcile all medications upon patient transfer between clinical services
  • Review daily therapeutic monitoring information for all patients served by the patient care unit (and other patient care units covered by the pharmacist's health care team)
  • Identify high-risk medication therapy by defined admission criteria and review as appropriate
  • Complete required competency exams as required by department

  • Provides Transitions of Care - Specific Clinical Pharmacy Services
  • Assist in evaluation of medication reconciliation, discharge pending and discharge reconciliation metrics
  • Optimize workflows for discharge pending communication and documentation
  • Evaluate hospital safety events related to transitions of care and work with administration to develop error reduction plans

  • Documents Patient Care Activities Performed
  • Document medication error information into standard reporting system
  • Identify, evaluate, and take steps to avoid adverse drug reactions on all patients
  • Update patient allergies and patient-specific height and weight information when needed
  • Review and ensure correct medication histories are entered into the medical record
  • Provide and document medication counseling to patients for targeted populations
  • Document therapeutic drug monitoring assessments and recommendations

  • Coordinates Patient Communications/Interactions
  • Perform medication history and reconcile home medications with inpatient orders
  • Discuss drug allergy history
  • Conduct patient teaching classes (where appropriate)
  • Assist with discharge medication reconciliation and pending for targeted populations
  • For targeted high-risk populations, provide medication counseling at discharge

  • Spearheads Patient Care Initiatives
  • Coordinate medication orders written during assigned hours of duty
  • Follow-up on medication system problems
  • Carry out, thorough interventions and education, protocol antibiotic and DUE initiatives
  • Participate in multi-disciplinary initiatives
  • Serve on intra- and inter-departmental committees as required
  • Coordinate discharge planning activities

  • Provides Education Services

    University of Iowa Health Care is a teaching institution; therefore, the clinical pharmacist is responsible for the education of students, residents, fellow staff, other healthcare providers, and patients about drugs and their appropriate use. The clinical pharmacist shall serve as a role model for staff and students. The educational process crosses professional boundaries, as the clinical pharmacist interacts with and serves as an informational resource for other members of the health care team. The clinical pharmacist takes great care to instruct all patients about the need for and appropriate use of prescribed medications. Examples of educational services may include:
  • Serving as preceptor for pharmacy students and pharmacy residents
  • Presenting in-service educational programs to staff nurses, physicians, and other colleagues on the healthcare team
  • Presenting at and/or participating in other educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UIHC or UI departments, and state and national organizations
  • Conducting educational classes for pharmacy students and pharmacy residents
  • Conducting patient education classes about the proper use of medications
  • Serving as mentor and role model for other pharmacist practitioners within the department

    Education Requirements:
    • Pharm. D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy
    Experience Requirements:
    • One (1) year of applicable pharmacy experience is required. Years of applicable experience include the time spent completing an ASHP-accredited pharmacy residency.
    • Current licensure by the Iowa Board of Pharmacy Examiners is required. Includes those eligible for obtaining this licensure.
    • Excellent written and verbal communication skills
    • Job related experience with and/or commitment to service excellence
    • Job related experience with and/or commitment to diversity in the work environment
    Desired Qualifications:
    • Experience in hospital pharmacy setting is highly desired
    • Basic Life Support (BLS) certification or eligibility for completing BLS certification
    • Satisfactory completion of an ASHP-accredited PGY1 residency in pharmacy
    • Satisfactory completion of an ASHP-accredited PGY2 residency in a specific field
    • Clinical experience in providing pharmaceutical care to patients in internal medicine areas
    • Board certification in Pharmacotherapy or another specialized field
