Description:
The Judge Group is looking for a Behavioral Health Case Manager to work in Langhorne, PA.!

Mon-Fri, day hours

On call (on call phone kept on weekly basis and rotates approx 1 x month)

Will use provided vehicle for in-person appts

Valid Drivers license and insurance required

Will take client (AKA patient) to Dr appts, grocery shopping, job interviews, appointments, etc in Lower Bucks County area

Responsibilities:

• Engage with and advocate for individuals diagnosed with mental illnesses and assist individuals in accessing services and supports including: mental health services, substance use services, housing
• and basic living needs, medical care, education, and employment.
• Develops an individual service plan with each individual, at least once every 6 months, which focuses on consumer strengths, interests, and goals, in order to encourage consumers to utilize
• available resources.
• Spends the majority of the workday providing face-to-face contacts with individuals and families in the Lower Bucks County community.
• Coordinates services provisions obtained through, but not limited to, MH programs, hospitals, vocational and educational providers, residential rehabilitation facilities, and participates in
• treatments planning meetings.
• Provide support and assist with resolution during crisis situations.

Qualifications:

• Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or
• education.
• Valid driver's license and ability to drive company vehicle.
