GENERAL SUMMARY/ OVERVIEW STATEMENT:

The Care Coordinator for MCPAP for Moms, in conjunction with Beacon Health Options, helps to evaluate the mental health, substance use and other support service needs for eligible providers and their patients struggling with perinatal behavioral health issues. The Care Coordinator performs day-to-day clinical activities and triage functions for MCPAP for Moms. The Care Coordinator also serves as a liaison for providers and patients for community behavioral health resources. The Care Coordinator works collaboratively with other MCPAP and MCPAP for Moms team members across Massachusetts to share resources, support one another and implement strategies.

PRINCIPAL RESPONSIBILITIES AND DUTIES:

  • Triages each service request from medical practices, providers and patients based on patient's needs, provider's reported ability to care for patient's needs and established program guidelines.
  • Communicates actively with off-site team members when appropriate while working independently on both the provider phone line and managing own case load for assigned region(s) across Massachusetts.
  • Gathers necessary clinical information from referring providers and translates information promptly and succinctly to psychiatric team members to help clarify the needs request prior to phone and face-to-face consultation services.
  • Offers coverage support to all Care Coordinator team members throughout the state by covering both provider phone shifts, and resource and referral case needs in any region of the state to make sure patients and providers receive MCPAP for Moms services in a timely manner.
  • Outreaches to patients according to program guidelines to discuss medical provider's requests, available local treatment options and listen to the patient's choice of care.
  • Educates each patient about local outpatient behavioral health options to help reach an informed decision about care needs and allow the patient's voluntary participation.
  • Evaluates potential good fit referral options for each patient in their home area. Independently weighing out the patient's needs with available mental health and substance use resources.
  • Completes follow-up outreach to patients to continue treatment needs assessment and guide patients to good fit options.
  • Creates individualized behavioral health resource lists for providers to share with patients in need of preventative care.
  • Works with team members throughout the state to help build, maintain, and update the program's mental health and substance use outpatient and support group search engines to help keep up-to-date records that allows for more efficient good fit search options.
  • Assists psychiatric team members to train medical practices.
  • Helps support and offer peer training to new Care Coordinator team members and orientation needs for new psychiatric team members.
  • Contributes recommendations for daily workflow and special projects.
  • Assists in development and maintenance of program guidelines.
  • Helps in the development of program materials.
  • Attends and participates in team meetings to review clinical cases as well as policy updates and programmatic changes.
  • Attends a variety of meetings, conferences and seminars for perinatal mental health education and program engagement needs.
  • Performs resource and referral duties in a HIPAA compliant manner, including correspondence with providers and patients regarding clinical and referral updates.
  • Maintains HIPAA compliant case file documentation both in electronic and paper format in accordance with programmatic standards.
  • Demonstrates use of quality improvement in daily operations.
  • Ensures highest level of service and satisfaction for medical providers and patients.
  • Complies with established departmental policies, procedures, and objectives.
  • Respects diverse views and approaches of team members, providers, and patients.
  • Contributes to maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
  • Complies with all health and safety regulations and requirements.
  • Performs other similar and related duties as required or directed.

Qualifications
SKILLS/ABILITIES REQUIRED:
  • Knowledge of medical terminology, health and wellness and healthcare office experience.
  • Preferred experience in issues of women's health and/or mental health.
  • Demonstrated expertise utilizing various software applications, including Microsoft Office applications, word processing, spreadsheet, presentation graphics, database utilization and medical system software, EPIC preferred.
  • Ability to work independently while prioritizing workload. Excellent verbal and written communication skills required.
  • Comfort and ability to deliver presentations to health care professionals.
  • Strong attention to detail and ability to maintain confidentiality.

QUALIFICATIONS:
  • Bachelor's degree in social work, psychology, or related field.
  • Minimum of four years related work experience.
