Summary: The APD/QIDP supports the Director of Residential Services and is responsible for providing oversight of the organizational, programmatic and financial elements of all services provided. The APD/QIDP will use sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards.
Education: Bachelor degree in social service field or a relevant degree.
Experience: Three years of progressive employment experience working with individuals with intellectual or developmental disabilities. Training and/or experience in developing and implementing active treatment programs, direct service delivery, staff training and case management.
Supervises: House Managers and Direct Care Staff

Essential Functions:
1. Facilitate and/or supervise programming and treatment of service population.
2. Perform the various duties required of a QIDP including developing, monitoring and revising active treatment goals and objectives; conducting annual Interdisciplinary Team Meetings (IDT) and interim IDT meetings for residents; writing quarterly, interim, and annual reports for each resident.
3. Ensures compliance with ICF-IID contract standards, state licensing requirements, regulating/accrediting bodies and BVT policies/procedures.
4. Ensures that all documentation is completed appropriately and in a timely manner and client documentation in case files.
5. Participates and assists in in-service training of staff.
6. Provide appropriate social services in accordance with licensing standards, contract terms, agency policies, ICF standards and funding requirements.
7. Supervise, facilitate and participate in the resident assessment and admission process.
8. Supervises and monitors staff in delivery of quality therapeutic services to clients/residents and discharge planning.
9. Interacts with state surveyors and resolves deficiencies by assisting Director of Residential Services in writing and implementing all Plans of Corrections cited in state surveys residential programs.
10. Develops Corrective Action Plan for any identified areas of need for direct reports.
11. Assists in the agency Quality Assurance Program through monitoring and case file audits.
12. Prepares and submits Incident Reports, Reports of Abuse, Neglect and Exploitation and submits reports to appropriate authorities.
13. Maintains Survey Notebook.
14. Monitors program through frequent observation and evaluation of programs services, staff, building, grounds, equipment, clients, etc.
15.Observes residents in each home during self-administration of medications during medication administration at least once per month.
16. Responds to outside requests for persons authorized to receive information regarding the residents.
17. Provides and/or supervises crisis intervention in crisis situations as needed (assisting with a combative client, potential suicide attempt, runaway, angry family, etc.).
18. Serves on internal and external committees with approval from Director of Residential Services or as assigned by the Executive Director.
19. Prepare and maintain department reports as assigned.
20. Coordinate with parents and guardians therapeutic home visits or absences for each assigned resident, update medical staff, house managers, Day Hab and Program Director of all assigned residents off campus arrangements with parents or guardians.
21.Responsible for timely completion of assignments and adapts schedule to changing workloads to include work in evenings, weekends and holidays when necessary.
22. Must maintain current license/certification/training in Texas.
23. Participate in weekly face to face or phone supervision with the supervisor.
24. Meet all predetermined deadlines and reporting requirements for the position.
25. Attend all meetings as assigned by Director of Residential Services
Supervisory Functions:
1. Be a key member in hiring, training, and terminating personnel as appropriate.
2. Provide consultation, feedback, evaluation, motivation and direction to staff, conducting staff meetings and communicates with staff on regular basis.
3. Implement disciplinary measures, including writing Employee Development Plans, as well as terminating employees as needed.
4. Assists the Director of Residential Services with maintaining annual training for direct reports.
5. Approve and enter time as required in ADP or agency time keeping software.
6. Monitor and approve requested time off from employees as needed.
7. Remain available to direct reports to assist with scheduling and ensuring appropriate coverage is maintained.
8. Ability to exercise discretion and independent judgement.
9. Supervises two (2) or more personnel as a regular job function.
Measurable Deliverables:
1. Residents are appropriately placed and individual plans for resident care outcomes are developed and tracked.
2. Programs maintain compliance with licensing standards and compliance documentation is properly prepared and submitted in a timely manner.
3. Develops, coordinates and monitors Active Treatment programs for assigned residents on case load working in close consultation with Inter Disciplinary Team (IDT) in case management of residents.
4. Weekly consultation with assigned House Manager to ensure service and deliverables are being met.
5. Assist each resident toward the achievement of active treatment goals.
6. Supervise residents in each home during self-administration of medications, as needed and document on agency approved documentation to include active treatment as appropriate.
7. Document resident behaviors on appropriate forms.
8. Assist House Manager with conducting required fire drills and emergency preparedness drills on an as needed basis.
9. Maintain BVT professional and ethical standards of conduct outlined in the employee handbook including demonstrating respect for agency staff, the service population, and community members and complying with required dress code at all times.
10. Meet all federal and state regulatory guidelines and standards that are applicable to this position.
11. Maintain a case load in accordance with agency policies and procedures and licensing and contract standards including meeting with residents on regular bi-weekly scheduled appointment.
12. Participate in workshops, seminars, education programs and other activities that promote professional growth and development.
13. Maintain a minimum number of weekly contacts with residents and families.
14. Continually assess ongoing changes in behavior, circumstances or conditions that may affect resident safety.
15. Provide feedback and support to ensure an appropriate response to calls, families' needs are addressed through best possible supports, and follow ups are conducted as needed.
16. Engage and involve resident and their family/legal guardian in the casework process.
17. Document progress that resident and their family/legal guardian toward the achievement of service goals and positive case outcomes.
18. Compile, prepare, submit and maintain accurate records, files, forms, statistics and additional information in accordance with agency policies, licensing and/or contract requirements.
19. Participate in Administrator on Duty Responsibilities, involving handling after hours calls or crisis situations in order to provide guidance to personnel, family members, residents, etc.
20. Work evenings, weekends and holidays to ensure that coverage is provided on an as needed basis.
21. Implement BVT safety protocols including evacuating with residents and other staff in case of an emergency.
22. Maintain confidentiality in all areas of the service population and program operations.
Other Responsibilities:
1. Perform other job duties as assigned by Director of ICF-IID Services.
2. Attend Psychological appointments if RN is unable to attend.
Credential, Skills and Abilities:
1. Maintain current licenses and certificates required for the position including but not limited to: First Aid, CPR, Emergency Behavior Intervention, etc.
2. Possess a valid Texas drivers' license, work eligibility status and have a good driving record.
3. Provide proof of work eligibility status upon request.
4. Must pass all pre-employment background checks including: Criminal History, Employee Misconduct, Nurse's Aide Registry, driving check and pass annual background checks.
5. Pass a pre-employment drug screen and random drug screen throughout employment.
6. Knows the indicators of abuse, neglect, and exploitation, and appropriate reporting and documentation procedures.
