This position will be required to work in our Southwest region (Burr Ridge & Tinley Park)

The Digestive Health Advanced Practice Provider ensures that care delivered to patients exceeds the standard of care and is provided according to Loyola Medicine's mission, strategic plans and all internal and external regulations and policies and procedures.

This role will be an outpatient Gastroenterology provider, working in the "south/southwest" ambulatory region. The provider will be in clinic both independently, as well as paired with a physician. Provider will work out of 2 locations and will see patients four days a week with one day of administrative time. The Advanced Practice Provider functions independently and collaboratively with the physician to provide continuity of health care services for individuals, families or groups.

Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment. Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, identifies variations from the norm in diagnoses emergent problems and complications.

Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution. Coordinates care for optimal continuity of care across the health care continuum.

Facilitates patient/family participation in health care by identifying learning needs and providing specialized education to enhance decision making.

Exhibits professionalism by performing research, education, and leadership in clinical practice.

Requirements:

Masters Degree - Nursing or Physician Assistant

CPR/BLS

APN: Nursing - Current Registered Nurse license State of Illinois Nursing - Nurse Practitioner license State of Illinois
