The Advanced Practice Nurse (APN) APN will work at ambulatory care sites within the Loyola Health System. The successful candidate must be skilled in diabetes management including insulin pumps, glucose sensing and diabetes medication adjustments. APN will also evaluate and treat other endocrine disorders including thyroid disease and metabolic bone disease. This provider will also work closely with the Inpatient Endocrinology Consult service by seeing patients soon after hospital discharge to establish their follow up care in the outpatient setting.

Ensures that care delivered to patients, meets/exceeds standards of care and is provided according to Loyola University Medical Center's mission, strategic plans and all internal/external regulations, policies and procedures. Practices within the Illinois Department of Professional Regulation Nurse Practice Act. Supports the established care models of Loyola University Medical Center.

Functions independently and collaboratively with the physician to provide continuity of health care services for individuals, families or groups.

Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment.

Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution.

Facilitates patient/family participation in health care by identifying learning needs and providing specialized education to enhance decision making.

Exhibits professionalism by performing research, education, and leadership in clinical practice.

Position Requirements:

Minimum Education:

Required: Masters Degree OR equivalent training acquired via work experience or education

Preferred: Masters Degree

Specify Degree(s): Social Work or Behavioral Health related field.

Minimum Experience:

Required: 3-5 years of previous job-related experience

Details: Minimum one year experience in completing Behavioral Health level of care assessments for adolescents and adults required. Minimum two years of progressive Utilization Review/Management experience in a Behavioral Health setting.

Licensure/Certifications:

Required: CPR (Cardiopulmonary Resuscitation)

Current Registered Nurse License State of Illinois

Licensed Clinical Social Worker State of Illinois
